Person-Centered Planning is an evidence-based practice used throughout the world to assist individuals in planning their lives and supports. The principal lies at the core of our service and informs much of our practice. When entering our program, individuals and their authorized representatives meet with our Qualified Intellectual Disability Professional (QIDP) to discuss the resident’s current outcomes and personal interests. This constructs an understanding of how our service can further develop this individual and informs our QIDP on how to develop a plan of supports. Through these discussions, our QIDP can create an Individualized Service Plan (ISP) which clarifies the outcomes that HRRS shall work on with the resident and provides instructions on how achieve these results. This plan is then disseminated to our team of Direct Support Professionals who begin to provide our service based on these guidelines. Essentially, each resident’s choices dictate the service that we provide and the Individualized Service Plan supplies the framework from which we operate to deliver this service.
